We encourage you to contact your surgeon and your insurance company if you have any questions about your financial obligations. In addition, please make sure your authorization/pre-certification for surgery is on file prior to surgery. We now offer online bill payment with Patient Notebook.

Physicians Day Surgery Center strives to remain transparent in the charges you may incur while receiving your care. Upon request, you will receive an estimate of charges based on the procedure(s) your physician has scheduled with us. Your actual charges will only be generated after the procedure(s) is complete to ensure it is accurate to what was performed.
Unless otherwise noted, your estimate of charges will include only the fees that will be generated by Physicians Day Surgery Center. Services may be provided in this health care facility by providers who may separately bill the patient, and who may or may not participate with the same health insurers or health maintenance organizations as the facility. You will receive additional charges from your physician. Depending on the procedure you are having you may also receive additional charges from:

Patients and prospective patients may request from this facility, and other health care providers, a more personalized estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in Physicians Day Surgery Center to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider.
If you have insurance coverage and we are contracted with your payer, the amount you will be personally responsible for will be determined by your coverage limits, deductible status, etc . We will make every attempt to determine your benefit status in advance in order to give you adequate information regarding your anticipated contribution. If you would like a written statement of your scheduled procedure(s) before your visit, or your actual performed procedures after your visit, please contact the billing office at 239-596-2557, Ext 105.
We do participate in many insurance plans; however, coverage changes occur frequently. If you are uncertain if we are participating in our network, please contact us directly or contact your health insurance carrier.
If you do not have health insurance, you will be responsible to pay for your care when it is rendered. We do not extend credit, however, we have partnered with ePay Healthcare to provide patient financing. Please call us 239-596-2557 ext 105 if you are interested in more information.
